The ROVER Team (Rapid on Site Virtual Essential Response) is a service which monitors and supports patients remotely at home once they have been discharged from hospital.

The service will provide patients with a daily call for 7-10 days following discharge to check on and monitor any medical concerns and to ensure that the patient has relevant supports in place, including follow up with a GP.

There is no fee for the service.

The service operates Monday – Friday, 8.30am – 4.30pm.

How does this service assist?

This service is focused on ensuring that your discharge from hospital is smooth and that you feel supported once you return home.

The service will provide you with the opportunity to speak with a nurse each day to discuss any ongoing symptoms or concerns that you have.

They will also be able to provide you with information about your condition and will help you to understand and manage the symptoms you may experience.

Who is GV Health’s ROVER service available to?

The program is aiming to focus on vulnerable and/or complex patients including those who are:

  • Living alone / socially isolated
  • Medical/Respiratory patients
  • Complex Medical and Chronic Disease including newly diagnosed Diabetes
  • Limited social supports


There are some people whom at this stage we cannot provide support to. This includes:

  • Residents of residential aged care facilities
  • Living in Supported Residential Service and disability accommodation
  • Department of Justice detainees (prisoners)
  • MCU@Home patients
  • Patients discharged from MCU
  • Patients on HITH
  • Paediatric Patients
  • Dialysis Patients
  • Oncology Patients

Access to GV Health’s ROVER service is via referral only. To arrange a referral, please speak to your treating team.
