Geri-Connect

Geri-Connect is a specialist geriatrician service for older people living in residential aged care facilities.

The service is designed to enhance, support and simplify clinical care through tele-health  consultations.

The service offers:

• Comprehensive geriatric assessment
• Timely review for complex cases
• Annual and medication review
  
  

The Geri-Connect project aims to:

• Provide more timely access to geriatric services to residents in aged care facilities
• Increase the number of older people receiving geriatric consultations at aged care facilities
• Help regional general practitioners (GPs) provide supported care to older patients living in aged care facilities
• Reduce polypharmacy
• Decrease rates of reduced mobility and falls
• Encourage greater use of allied health services
