The Aged Care Assessment agency is a team of trained non-clinical and clinical assessors who provide assessments to older people in our community, assist with information and advice to assist clients to make choices about their future lifestyle and the type of care or support that will best meet their needs.

Aged Care assessors conduct both comprehensive (clinical) and simple (non-clinical) assessments to determine eligibility for Australian Commonwealth funded services, including:

  • Community service options such as general home care,
  • Support at Home,
  • Home modifications,
  • Assistive Technology,
  • Residential care in an aged care home,
  • Residential respite care,
  • Transition Care Program,
  • Restorative care and
  • End of Life care

Who is an Aged Care Assessment available to?

An Aged Care Assessment is available to people aged 65 and over, and Aboriginal and Torres Strait Islander people aged 50 and over and meet eligibility criteria for an assessment.

The GV Health Aged Care Assessment team assist to service the Hume region in Victoria.

How do I access the Aged Care Assessment program?

Clients can refer themselves or be referred by a family member, carer, General Practitioner (GP) or health professional.

Referrals for an ACA assessment can be made via My Aged Care:

  • Call 1800 200 422, Monday – Friday, 8am to 8pm, and Saturday, 10am to 2pm.
  • Apply for an assessment online.
  • If you are deaf and/or find it hard hearing or speaking with people who use a phone, the National Relay Service can help you. Select your preferred access option and ask for 1800 200 422.
  • If you English is your second language, the Translating and Interpreting Service can help you. Call 131 450, tell the operator the language you speak and ask them to call 1800 200 422.

What happens during an assessment?

An assessment takes between one to two hours and the assessor will ask you questions about your health and wellbeing, including how you are managing and what help you are receiving.

The assessor will then discuss your goals with you to develop a care plan and determine the kind of care that best meets your needs.

A family member, carer or a friend are welcome to be present during your assessment and an interpreter can be arranged for you if necessary.

With your approval, the Aged Care Assessor may also contact your GP to gain more information on your medical history and help with the assessment process.

Assessments are conducted in client’s homes, residential aged care homes and in hospital. There is no cost for this service.
