The Self Management Support team provides individual support and group programs for people with ongoing (chronic) health conditions who want to make healthy lifestyle changes.
The aim is to build an individual‘s ability to self manage and increase their motivation and self efficacy.
Through a variety of programs we can help you:
- Set goals and make a plan to manage your health
- Improve your motivation
- Increase your physical activity
- Include more ‘me’ time
- Quit smoking
- Make healthier choices
Clients can refer themselves or be referred by a health professional.
