District Nursing Services
District Nursing Services provides general and specialised nursing and healthcare to clients to improve their health status and retain their independence to continue living at home – taking an active role in their healthcare.

Hospital in the Home
Hospital in the Home enables care to be provided at home for patients who would otherwise require hospital admission. Medical Case Management is provided by the Hospital in the Home Senior Medical Officer or a community GP linked with the program.
Regional Continence Service
Regional Continence Service provides specialist clinical assessment, diagnosis, management, education and support to improve continence for clients. The service provides consultancy, education and support to carers, relatives and professional service providers.
The Home Nursing Service has links with the West Hume: Regional Continence Consultant and Regional Wound Consultant. Contact the District Nursing Service for further information.
You may be required to pay a fee for your Nursing Service. This fee depends on the specialty nursing that you require, your individual circumstances and entitlements. This will be discussed with you. If you are requiring wound care you may be charged according to the products that are required for your care.
During your assessment the staff members will focus on how they can assist you to achieve your best health outcomes.
Assessments may include:
- A physical assessment
- Discussion regarding your goals for your health, and strategies to reach them
- Access or referral to other service providers
As our service is a home visiting service you will be asked a series of questions regarding visiting your home, prior to your first home visit.
We request that all clients refrain from smoking whilst we are visiting.
We request that all pets are restrained or kept in another area of the home when the nurse visits.
