The Care Coordination Team consists of 2 subprograms including:
Post Acute Care Program
Provides short-term care /service coordination and referral/information support to clients identified as having complex care needs either following discharge from a public hospital or at the time of an Emergency Department presentation.
Hospital Admission Risk Program – Emergency Department (HARP-ED)
Provides short term case management, education and referrals for clients with complex aged related or psychosocial issues.
These programs assist public patients identified as at risk of poor discharge outcomes without short-term care coordination and brokerage service support.
