The Hume Regional Aged Care Assessment Service – Shepparton (ACAS) is a team of experienced health professionals who provide assessment, information, advice and assistance to frail older people aged 65 and over or Aboriginal and Torres Strait Islander people aged 50 and over who want to remain at home or who are considering living in an aged care home.
People who are not aged are eligible for aged care services in some circumstances.
The team aims to assist people to make the best choice about their future lifestyle and the type of care or support that will best meet their needs.
ACAS assesses for eligibility for entry to
- Residential Aged Care Homes
- High & Low Level Residential Respite
- Home Care Packages, levels 1 & 2, 3 & 4 and
- The Transition Care Program.
ACAS – Shepparton services the Local Government Areas of Moira, Greater Shepparton, Strathbogie, Mitchell and Murrindindi.
ACAS conduct assessments in the community, Residential Aged Care Homes and in hospital.
There is no cost for this service.
Referral Criteria
- Client must have given informed consent to referral.
- Clients must be medically and psychiatrically stable.
- All restorative options must be completed.
- If client is able to return home after a hospital stay, refer on discharge except in the case of Transition Care.
- Access to the Transition Care Program is directly from hospital.
Referrals are registered and prioritised according to individual client need.
