West Hume Primary Mental Health & Early Intervention Team (PMH & EI Team)
The PMH&EI Team is a specialist multidisciplinary team, which supports the work of primary health care providers, including General Practitioners in the West Hume area. This support is offered through clinical support services, education and professional development activities and service development.
Clinical Primary Mental Health Services are provided to people of all ages, who are experiencing a high prevalence mental disorder, which includes but is not limited to, depression and anxiety related disorders. The Early Intervention Service aims to establish effective mechanisms to meet the needs of young people, aged 16-25 years, who are experiencing, or are at risk of experiencing a serious mental disorder, especially first episode psychosis.
Referrals for clinical services can be made by primary healthcare providers who are located in the municipalities of Mitchell, Murrindindi, Strathbogie, Moira and the City of Greater Shepparton. All clients referred to the team must have a nominated general practitioner who is aware of the referral and agrees to work with the Team in the delivery of the clinical service. Services are provided free of charge.
The Team also facilitates the provision of education and training activities to primary healthcare providers and community groups. These activities aim to raise awareness of mental disorders and provide general information regarding the early recognition and treatment of them. Service development activities aim to promote collaborative relationships between primary care agencies and mental health service providers to support the care of people who are experiencing a mental disorder.
Request for services can be made directly to the Team by phone, fax or directly to a member of the Team, however PMH&EI do require referral to be made by your GP.
