For entry into Grutzner House you need to
- Confirm a local GP
- Have completed ACAS Assessment (ACRE) determining high care current client of APATT with completed assessment and identified for respite or “permanent” placement
- Ideally visit and view facility and discuss possible placement with Nurse Unit Manager
- Complete the application and agreements
