Between them, these programs provide prevention and health promotion to support good health, early intervention services to lessen the need for more intensive services in the future, care in the home as an alternative to hospital based care where appropriate, support following hospital discharge to assist recovery and reduce the risk of re-admission to hospital and services to support independent living in the home environment for frail, aged and disabled people.
